Meaning of ER or Office Visit Due to Heart Failure
An event where the patient has an urgent, unscheduled office/practice or emergency department visit for a primary diagnosis of heart failure (HF) but is not admitted to the hospital, where the patient exhibits new or worsening symptoms of HF on presentation, has objective evidence of new or worsening HF, and receives initiation or intensification of treatment specifically for HF, with the exception that changes to oral diuretic therapy do no qualify as initiation or intensification of treatment.
